This map is continually being modified, given the scale of PHO mergers that are currently underway, and the emergence of various primary care neworks or alliances.  A number of PHOs remain on this map even though they may no longer hold a PHO contract and/or may be in the process of winding up, because the organisations still exist and, in some cases, may still be delivering or funding primary care services.
